II. TENURED PROFESSOR

  1. Five-Year Reviews

    Per APM 200-0, “Every faculty member shall be reviewed at least every five years.”  Please see The CALL, Appendix 12, III for those academic titles that apply.

    1. Submitting the Action
      Please submit an original plus one copy of the dossier to the L&S Personnel Office.

    2. Dossier Contents in the order below.  For the Five-Year Review, the minimum material within the dossier should include:  (Use the appropriate Data Summary pages from the L&S Website.)
      1. Five-Year Review Cover Page.
      2. History Record (updated by the department).
      3. Data Summary Sheets for 5 Year Review.
      4. Bibliography.
      5. Certification Page.  This page must be read and signed by the candidate
      6. Candidate’s written response to the departmental assessment (if submitted).
      7. Chair’s recommendation.

        Other Data Summary pages are at the option of the candidate/department.

        Publications and teaching evaluations:  These should not be submitted to L&S, but should be available on Dean’s request.


    3. Routing and Approval
      The L&S Personnel Office staff will review the dossier and forward it to the appropriate Dean for consideration.  See The CALL, Appendix 12, V for examples.  Per Appendix 12, VIII:
      1. Dean’s Final.  “Where the Dean is responsible for a final action in the Five-Year Review, the Dean, after considering the document(s) submitted by the Department Chair, determines the outcome of the review, and sends a letter reporting the outcome to the Department Chair with a copy for the faculty member.”
      2. Chancellor’s Final.  “Where the Chancellor is responsible for final action, the Dean transmits the material submitted by the Chair together with the Dean’s statement.  The Chancellor determines the outcome of the review, and sends a letter reporting the outcome to the Department Chair, with a copy to the Dean and a copy for the faculty member.”
