Since the SAAP program is funded through assessment of staff salaries, the requirements for the distribution of the funds are more rigorous than for other types of awards. We must ensure that we are following both UC, and UCLA/CHR, guidelines. Consequently, when you request approval for your nominees, you must submit the following:

• A description of your department's plan. This need not be elaborate, and can be as simple as an e-mail describing your criteria



• Your nomination and selection process. Again, this can be as simple as stating that the chair reviews all staff once a year and makes the determination him/herself.



• How and when the plan was communicated to eligible staff. This is important, as staff is entitled to know how they might qualify for such an award.



• The last page, signed, of a performance review, given to the employee for whom the request is being made, within the last 12 months.
